Support. Belong. Thrive.
Supportive Living with Purpose.
Established in 1985, Hilltop’s Life Adjustment Program (LAP) is the only supportive living community for adults with brain injuries on the Western Slope and is home to up to 90 individuals. Personalized, multidisciplinary care plans support each resident’s physical, emotional, and social wellbeing. Daily activities, compassionate staff, and a culture rooted in fun, acceptance, and meaningful connection create a true sense of belonging.

Our culture and care approach
Our Culture
- We’re driven by a shared purpose between both staff and residents.
- We aim to create engaging and trusting professional relationships with the individuals we serve, families, staff, and all entities that support our residents.
- We recognize each person’s individuality and respond with compassion in a way that validates their journey.
The Things We Focus On
- Treating individuals as “adults first” and focusing on holistic care and connection after surviving a brain injury.
- Working from a transparent, resident-driven, multidisciplinary comprehensive care plan with measurable and achievable goals.
- Supporting individuals as they navigate their goals and interests.
The 5 Things We Live By
- Generosity: setting boundaries while staying true to ourselves.
- Predictability: doing what we say we’re going to do.
- Consistency: providing consistency through boundaries, values, actions, and beliefs.
- Kindness: acting with compassion and empathy.
- Transparency: communicating openly and honestly.
Meaningful days. Stronger connections.
Engaging Daily Life
Our daily activities are designed to support wellbeing, encourage connection, and promote independence, with opportunities both on campus and throughout the community.
Outings in the Community
We enjoy getting out and exploring the Grand Junction area. Community outings provide opportunities to try new experiences, visit local businesses, attend events, and stay connected.
Activities on Campus
Our campus offers a welcoming environment where individuals can build skills, make friends, and enjoy a variety of daily activities.
Some activities may have an additional cost.
